\nOf the book covers you show, I prefer the last one, with the 4 symbols going across. My one recommendation would be to remove the complicated gradient\/pattern behind it and keep it clean and simple, with a warm but neutral color in the bg.

\nI love the overall framework of the storyboard. How are you thinking to produce this visually? With photography or illustration? I very much like the illustration style of the icons, so perhaps use this same style and illustrate your video? I know that's a lot of work, but it would match stylistically and illustration makes things more universal vs. personal. I imagine using your voice, some light happy music, and an illustrative style of video working very well.

\nMy only other concern is lack of detail of the UI\/UX in your storyboards. Other students have failed their reviews due to lack of detail of the user experience\/UI, and I don't want that to happen to you. Please include some screens of what the app looks like as someone is going through a health-related crisis or problem, and how the app is helping them.
